Patient App Terms
What you agree to when you create a MedTap account.
Version 1.0 · effective 6 September 2026 · Expy LLC, an Idaho limited liability company
1. Who provides the app
MedTap is provided by Expy LLC, business address to be added. These terms govern your use of the MedTap patient app. Your clinic provides your healthcare and has its own privacy notice, treatment documents, and payment policies. Accepting these app terms does not accept a clinical procedure or authorize an unrelated disclosure of your health information.
2. Who can use this pilot
This service is for adults who can legally agree to these terms and are using MedTap in the supported launch area. You may use MedTap to complete intake for a participating clinic and to keep your own MedTap record of the information you provide. Do not create an account for a child or another person through this pilot. Contact your clinic for an assisted intake option if you need help, a representative, language assistance, or an alternative to this app. This app limitation does not determine your eligibility for medical care.
Use accurate information and your own account. Keep your login private, sign out of shared devices, and notify support@expymed.com if you believe someone has accessed your account. Do not access another person's records without an approved, verified arrangement.
2A. Your MedTap record
Information you enter into your own MedTap record is held by Expy LLC for you, at your direction. It is separate from the record a clinic creates when it receives and files your intake.
You may reuse your MedTap record to complete intake at another participating practice: your saved answers fill in that practice's forms so you do not retype them. Nothing is sent until you review the completed intake and submit it, and we show you the practice it is going to before you do. We do not send your record to a practice without that submission.
Your MedTap record is not your clinic's medical record. A clinic keeps its own record of your care under its own legal obligations. Ending your use of MedTap, or a clinic ending its use of MedTap, does not change or delete the clinic's record.
You may export or delete your MedTap record at any time. Deleting it does not delete or recall anything already sent to and filed by a practice.
3. What the app does
The app lets you provide intake information to the clinic shown on the screen and review the submissions and documents made available to you. Review your information and the named clinic before submitting. Tell your clinic about corrections or changes that could affect your care.
The app does not provide medical advice, diagnose conditions, monitor emergencies, guarantee an appointment, or promise insurance coverage. For an emergency, call 911 or the appropriate emergency service. For questions about treatment, medications, appointments, or bills, contact your clinic directly.
4. Your information and choices
You retain your rights in the information you provide. You permit MedTap to process it as needed to provide the requested service, subject to applicable law, the MedTap privacy notice, and MedTap's agreements with your clinic. This is not a general license to sell your health information, advertise using it, or train general models on it.
The MedTap privacy notice explains the app's data handling. Your clinic's Notice of Privacy Practices describes the clinic's uses, disclosures, and your health-record rights. An acknowledgment that you received that notice is separate from a permission to share information for a special purpose. You may decline to acknowledge receipt; the clinic may document that choice.
Any treatment consent or separate authorization appears as its own clinic document with its purpose and terms. Optional communications and optional sharing require their own choices where applicable. You do not agree to marketing messages simply by creating an account or entering a phone number.
5. Electronic actions and copies
When you select a clearly labeled agreement or signature action, you intend to take the action described on that screen. The app identifies the document and the person or organization receiving it before you act. You may review and save a copy of documents you accept or sign. Ask your clinic for an alternative if you cannot use the electronic process.
Where the law requires separate consent to receive legally required records electronically, that consent and the required disclosures will be requested separately. These general app terms alone do not replace that process or the requirements for a particular clinical authorization.
6. Account closure and record requests
You may contact privacy@expymed.com to request account closure, a copy of account information, or help with a privacy request. We will verify the request appropriately. Closing an account does not automatically delete records your clinic must retain, change a previously delivered clinical record, or undo a disclosure that has already occurred.
For access to, correction of, or other rights concerning your clinic's medical record, contact the clinic. We will help direct requests concerning clinic-controlled information to the clinic and provide assistance consistent with our obligations. We will explain applicable retention and identity-verification requirements rather than promising deletion of every copy.
7. Fees, availability and misuse
There is no MedTap patient app fee under this pilot. Your clinic may separately charge for healthcare under its own policies. We will not charge you for a paid MedTap feature without first showing its terms and obtaining your agreement.
The app may be unavailable or contain errors. Use your clinic's alternative intake method when necessary. We may restrict an account to address misuse, unlawful access, or security risks, with notice when appropriate. Restrictions on app use do not remove medical-record rights under applicable law.
Do not upload unlawful content, attempt to bypass access controls, impersonate another person, or interfere with the service. We retain rights in our software; using it does not transfer those rights to you.
8. Changes and contact
We will explain material changes to these terms and ask you to affirmatively accept the new version before it governs future app use, where applicable. Earlier acceptance records remain available. New terms cannot retroactively authorize a new use of your previously collected information contrary to law or an existing restriction.
Nothing here waives rights or remedies that cannot legally be waived. These terms impose no mandatory arbitration, no class-action waiver, and no blanket release of liability.
Contact support@expymed.com for account help and privacy@expymed.com for privacy questions. The clinic's own contact appears with your intake and its privacy notice.
